Shift premium: $1.05 per hour for work after 3 p.m. $1.05 per hour for work after 11 p.m. Weekend premium: $1.05 per hour for work after 11:45 p.m. Friday until 11:45 p.m. Sunday. Supervisor shifts: $0.75 per hour.
Paid holidays: 12 days. (Previously 11 days)
Vacations with pay: First year worked: 1 day for 2 months’ service, 2 days for 3 months’, 3 days for 4 months’, 4 days for 5 months’, 5 days for six months’, 5 days for 7 months’, 6 days for 8 months’, 7 days for 9 months’, 8 days for 10 months’, 9 days for 11 months’. 2 weeks after 1 year, 3 weeks after 2 years, 4 weeks after 5 years, 5 weeks after 13 years, 6 weeks after 22 years, 7 weeks after 28 years.
Overtime: Time and one-half after 7.5 hours worked per day or 75 hours bi-weekly. Time and one-half for call-in work on a vacation day. Time and one-half for work on holiday. Double time and one-quarter for more than 7.5 hours per holiday.
Meal allowance: $7 cafeteria voucher or $5 cash for overtime of more than 2 hours per day.
Medical benefits: 100% covered by employer for semi-private, $3 co-pay for prescriptions.
Dental: 75% covered by employer. Orthodontic: 50% covered by employer, $1,500 lifetime maximum. Dentures: 50% covered by employer, $1,500 maximum every 5 years. Crowns, bridgework, repairs: 50% covered by employer, $1,500 maximum every 5 years.
Vision: $400 every 2 years. (Previously $200) $90 for eye exams every 2 years. (Previously $60)
Paramedical: Massage therapy: $400 per year. (Previously $300) Chiropractic: $400 per year. (Previously $300)
Sick leave: 3 days after 4 months’ service. 1 day per month thereafter. 1.5 days per month after 1 year’s service.
Life insurance: Employer pays premiums for Hospitals of Ontario Group Life Insurance Plan. Twice annual salary payout.
Pension: Healthcare of Ontario Pension Plan.
Bereavement leave: 4 days for death in immediate family (parent, spouse, child). (Previously 3 days). 3 days for death in extended family (sister, brother, stepmother, stepfather, stepchild, stepsister, stepbrother, mother-in-law, father-in-law, sister-in-law, brother-in-law, son-in-law, daughter-in-law, grandchild, grandparent). 1 day for death in other family (aunt, uncle, niece, nephew).
Call-in pay: Minimum 4 hours at time and one-half, or applicable premium rate, whichever is greater.
Probationary period: 450 hours.
Discipline: Sunset clause is 14 months.
Severance: 2 weeks per year, maximum 12 weeks.
Uniforms/clothing: Full-time workers $5 per month, part-time workers $2.50 per month if no uniforms are available from the employer. (Previously $50 per year) 2 coveralls and 5 uniforms (shirt, pants) for dirty work. Winter parka for outside work.
Tool allowance: $1,000 per year.
